Consultation Form

This form helps your care team have a plan in place before you ever show up! This form must be turned in to schedule your appointment!

Meet and greet appointment

This appointment we do a basic health check, go over your form, and make a plan for our major appointment

Homework and practice

From your first appointment you might have homework to practice and get ready for your next big appointment

Second appointment

This is where we do any blood work or big tests or anything we didn’t get done during the first appointment!
